CME: Updates in Alzheimer Disease (AD) and Its Complications: Pharmacotherapy and Barriers to Optimal Care

ACCREDITATION EXPIRED: July 07, 2020

Activity Description / Statement of Need:

In this online, self-learning activity:

Alzheimer Disease (AD) is a degenerative disease that most commonly affects the elderly, although it is occasionally detected as early as middle age. AD accounts for over half of all diagnosed dementia, the prevalence of which is increasing. Once there is a diagnosis of probable AD, one must determine which pharmacotherapy, if any, is most appropriate for treatment of the patient. The literature suggests that gaps in care on the part of healthcare professionals exist. While some physicians continue to use the antiquated, nonspecific term “senile dementia” as a descriptive diagnosis of AD, perhaps suggesting a lack of understanding of the gravity of the disease state or its associated pathophysiology, other report feeling uncertain at times about now to best diagnosis of the disease, particularly in its very early stages. Moreover, a number of different practice guidelines have been updated recently, and HCPs are often unable to keep up with the publishing of literature and evolution of clinical practice.

Target Audience:

Healthcare professionals specializing in: neurology, gerontology, internal medicine, palliative care, or those who otherwise commonly care for patients with AD or who frequently encounter them or their caregivers in practice.

This program is supported by an educational grant from AVANIR.

Release Date: July 07, 2018 -- Expiration Date: July 07, 2020

Faculty: Mario Mendez, MD

Agenda

Introduction content: cursory refresher and review of AD

  • A broad overview: AD within the context of dementia
  • Diagnostic criteria, imaging, biomarkers, and screening
  • Pathophysiological process
  • Epidemiology: prevalence of AD vs. dementia overall
  • Risk factors (e.g., cardio/cerebrovascular, Gaucher’s disease, Down syndrome, genetic factors, family history, biochemical, head trauma)
  • differential diagnosis and disease progression)
  • Presentation (motor symptoms), evaluation, and diagnostic challenges
  • Patient case(s)

Treatment of AD

  • Hypotheses underlying present pharmacotherapy and mechanisms of activity
  • Present pharmacotherapy
  • Comparative efficacy and evidence update
  • Treatment-refractory disease and complications
  • Management of motor fluctuations and dyskinesia
  • Behavioral and other disturbances in dementia: psychosis, depression, mood disturbances, dementia, sleep disturbance, sensory derangement, impaired cognition, and autonomic defects
  • An examination of AD psychosis
  • Antipsychotic use in dementia
  • Antidepressants in dementia
  • Other clinical pearls
  • A distinction between disease progression and adherence to therapy
  • Emerging and future therapies: aducanumab, AMG 502 / CNP520, baclofen-acamprosate, idalopirdine, lanabecestat, solanezumab
  • Adjunct considerations
  • Alzheimer medical foods
  • Preventive care
  • Nonpharmacological therapy, including psychosocial management
  • Barriers to care: optimizing patients’ adherence to their therapy and minimizing adverse effects
  • Patient case(s)

Learning Objectives

By the end of the session the participant will be able to:

  • Describe validated diagnostic and screening method(s) for AD and apply them to a patient case
  • Discuss the neuropathophysiology of AD and how it relates to presently available AD treatments mechanisms
  • List present and emerging pharmacotherapeutic therapies for the treatment of AD and its complications and apply them to patient cases, taking into account comorbid disease states and drug therapies
  • Describe the impact of medication adherence on patient outcomes and formulate strategies to improve adherence in patients with AD
