Activity Description / Statement of Need:
In this online CME self-learning program: The overall learning activity will focus on Anticoagulant and Antithrombotic therapy, and the role of the physicians and allied health professions play in it. Anticoagulant therapy remains a common source of medication errors. As a current and longstanding National Patient Safety Goal, anticoagulation therapy is recognized as engendering gaps in practice.

• What happens when we give too much? A rational approach to reversal
• What happens when we give too little? Missed opportunities in the prevention of thromboembolic events
• Special challenges relating to perioperative anticoagulation bridging and neuraxial blockade
• A rational approach to reversal agents and how we learn from them

Learning Objectives
By the end of the session the participant will be able to:
- Describe the systems factors that play a major role in patient safety challenges related to anticoagulation therapy.
- Apply current National Patient Safety Goals to sample scenarios, recommending strategies for improvement.
- Given clinical scenarios, identify how approaches to anticoagulation and its reversal could be improved at the system level, integrating health informatics and clinical decision support.
- Describe the role of patient adherence in anticoagulation-related readmission and other outcomes, describing the role that health informatics and clinical decision support may play.
- Design a strategy to effectively educate healthcare professionals about special considerations for the storage, handling, and administration of different anticoagulants.
